Proforma / Letter to an employee who has remained absent but reported for duty along with Medical Certificate

You are working on a responsible position as ………………………. in this organization.

As an employee working for considerable period, you are very well aware about the leave sanctioning procedure followed in this organization. However, ever since______________ (Date), you have been absenting yourself in an unauthorized manner i.e. without obtaining prior intimation or sanction /  approval for leaves or even application to this effect. Accordingly, such absence from duty has been termed as unauthorized and amounted to serious misconduct as per the service rules applicable to you in this regard.

However, on __________ (Date), you have reported for duty along with a medical certificate but the same would not justify for sanction of leave since it was to be sent on falling sick and not at the time of resumption of duty.

In view of the above, your willful lapse is of serious nature and amounts to a misconduct as per service rules applicable to you. Without prejudice to the right of the management a lenient action will be taken by allowing you to resume duty whereas your absence will not be treated as leave /as a special case it has been treated as leave. Should you repeat any such lapse in future. management will leave with no option but to take strict disciplinary action against you.
